Hearts+ is a heart-failure self-care platform. Enrolled participants can receive text messages from SAHRA, our care assistant: medication reminders, check-ins, and replies to their questions. Texting is optional and never required to receive care. Message frequency varies, and message and data rates may apply. Reply HELP for help or STOP to opt out at any time.
