How patients opt in to Hearts+ text messages

What this page is

Hearts+ is a heart failure self care platform for adults enrolled in a supervised care and research program. SAHRA, the assistant inside it, can support patients by text message if they ask for it.

Patients consent to text messaging either in person with their clinician or inside the Hearts+ app after signing in. Neither place can be reached by someone without an account, so this page publishes the full consent experience, exactly as a patient sees it, for review.

Texting is optional

Agreeing to text messages is never a condition of enrolling in Hearts+, of receiving care, or of using any other part of the platform. Patients who decline can use the Hearts+ mobile or web app instead, and declining changes nothing about their care or their participation in the study. There is no public web form that collects phone numbers, and no number is ever messaged without one of the two consent paths below.

Path 1. In the Hearts+ app

A signed in patient opens Settings, reads the text messaging disclosures, selects the consent box, and enters their own mobile number. The consent box is not selected by default, and Verify by SMS stays unavailable until the patient selects it. The patient then confirms a one time code sent to that number before any other message is sent. Mobile numbers are redacted in the screenshots below, which were taken on a test account.

Path 2. In person at enrollment

During enrollment, a clinician or study coordinator reviews a written text messaging authorization with the patient. It carries the same disclosures shown in the app, and the patient chooses whether to accept it. Nothing is selected for them. A patient who accepts receives one confirmation message and nothing further unless they reply YES.

What SAHRA sends

Messages are one to one with the individually enrolled patient and support their care plan. We never send marketing or promotional text messages. These are the real messages the service sends:

  • Welcome to Hearts+. SAHRA helps you manage your heart care by text. You can also call this number anytime to talk with SAHRA. Reply YES to begin.
  • Great. Create a 4-digit PIN you will use to view your health info. Reply with 4 digits.
  • 1 Log something 2 My health info 3 Talk to SAHRA. Text MENU anytime. Text PIN to reset your PIN.
  • Time for your evening heart medication. Reply 1 Yes, 2 No, or 3 to talk to SAHRA.
  • Logged: weight 82 kg. Text MENU for options.

Opt out keywords are STOP, STOPALL, UNSUBSCRIBE, CANCEL, END, QUIT, OPTOUT and REVOKE. Help keywords are HELP and INFO. A patient can also turn text messaging off in the app under Settings, or ask the research team to turn it off for them. Stopping text messages never ends their participation in Hearts+ or affects their care.

Privacy

We do not sell mobile phone numbers, and no mobile information is shared with third parties or affiliates for their own marketing or promotional purposes. A patient number is used only to deliver their Hearts+ care messages, and is disclosed only to the messaging providers that carry those messages, under Business Associate Agreements and only on our instructions.
